Kabar dari Itali: Kisah Perjalanan Covid-19 di Italia

TAHAP 1

Tanggal 20 Februari 2020 adalah awal di mana Pemerintah Italia mulai menghitung kasus covid19 di Italia, yaitu sejak dinyatakan positifnya seorang pria Italia berumur 38 tahun di kota Codogno di utara Italia. Sehari setelahnya, pemerintah setempat melakukan lockdown terhadap 14 comune (selevel kecamatan di Indonesia) setelah terjangkitnya 16 orang dan 1 diantaranya meninggal.

Pada tanggal 5 Maret, pemerintah mulai menutup sekolah dan kampus, dimana awalnya hanya direncanakan hingga 15 Maret. Namun, pemerintah justru melakukan karantina terhadap Regione Lombardia (yang ibukotanya Milan), bersama 11 Provinsi lainnya (selevel kabupaten/kota) di utara Italia terhitung sejak tanggal 8 Maret. Lebih dari 16 juta orang terkarantina ketika itu, dimana jumlah terjangkit sudah mencapai 7,357 kasus, dan 366 orang meninggal. Pada masa ini seluruh aktifitas yang berpotensi mengumpulkan orang banyak telah dihentikan, seperti aktifitas rumah ibadah, dan lain sebagainya.

Kemudian, pada tanggal 10 Maret Italia resmi melakukan karantina total dengan menetapkan zona merah bagi seluruh wilayah Italia yang awalnya direncanakan berakhir pada tanggal 3 April. Sehari setelahnya pemerintah pun menutup restoran, bar, café, dan tempat-tempat lainnya, sehingga hanya menyisakan groceries dan apotik saja. Kebijakan lockdown ini terus berlanjut dengan diperpanjang sebanyak dua kali dari yang awalnya tanggal 13 April dan terakhir 3 Mei.

Ada beberapa kejadian menarik di tahap awal ini, yaitu diantaranya adalah terjadinya gejolak terhadap kebijakan lockdown yang dilakukan. Beberapa gubernur dan walikota dikabarkan memprotes kebijakan tersebut, pihak oposisi pemerintah pun melakukan hal yang sama, bahkan meminta klarifikasi lebih lanjut atas kebijakan karantina wilayah yang diambil. Masyarakat Italia yang terkenal dengan budayanya yang hangat serta mengusung kehidupan yang bebas juga banyak yang memprotes kebijakan mengekang tersebut, mereka membutuhkan waktu lebih lama untuk akhirnya bisa taat dan patuh atas aturan tersebut. Hal ini merupakan salah satu factor tersebar luasnya covid-19 di masyarakat Italia.

Kejadian berikutnya adalah keluarnya orang-orang dari Lombardia sejak tanggal 9 Maret (sehari setelah lockdown Lombardia). Bahkan seminggu setelah lockdown diterapkan, orang-orang masih bisa keluar dengan leluasa dengan menggunakan kendaraan pribadi, karena yang sudah mulai di awasi ketika itu adalah kendaraan umum. Kejadian ini adalah sebab lain tersebarnya covid-19 di seluruh wilayah Italia secara luas, karena “mudik” yang dilakukan orang-orang dari utara Italia menjadi sarana penyebaran wabah ke daerah-daerah baru yang sebelumnya masih relative aman.

Dalam masa satu minggu setelah karantina wilayah, polisi memeriksa sekitar 700.000 orang di seluruh Italia, dan 43.000 diantaranya ditetapkan untuk dikenakan denda karena keluar rumah tanpa ada alasan yang jelas. Hal ini menyebabkan pemerintah harus menaikkan jumlah denda yang awalnya 260 euro, menjadi 400-4.000 euro (tergantung pelanggaran)

Satu hal yang menarik adalah, PM Italia selalu berkonsultasi dengan para ilmuan serta ketua serikat bisnis dalam mengambil kebijakan. PM Conte mengatakan bahwa dia mengambil tanggung jawab politik secara penuh terhadap semua keputusan yang diambil.

Hingga saat ini sudah ada total 197.675 kasus terjangkit, 26.644 kasus meninggal, dan 64.928 kasus sembuh, sehingga masih menyisakan 106.103 kasus aktif. Angka ini cukup besar, mengingat sebanyak 1.757.659 swab tes (dan uji PCR) telah dilakukan, dan 11.25% diantaranya dinyatakan positif. Selain itu, jumlah orang tua yang besar juga berkontribusi besar terhadap jumlah kasus meninggal, dimana seperempat dari penduduk Italia adalah berumur di atas 65 tahun (sebagaimana yang diketahui 85% kasus meninggal berumur diatas 70 tahun)

MENUJU TAHAP 2

Tanggal 26 April, PM Conte mengumumkan tentang rencana Italia memasuki tahap 2, yang akan dimulai pada 4 Mei mendatang, yaitu “melonggarkan lockdown menuju social distancing”, atau membuka lockdown dengan tetap menjaga prinsip menjaga jarak antar orang per orang.

Beberapa kelonggaran yang akan diterapkan per 4 Mei diantaranya adalah (1) mulai dibolehkannya kunjungan individu kepada kerabat dengan tetap tanpa bersentuhan (memeluk); (2) diijinkan kembalinya para pekerja dan/atau perantau yang sebelumnya dilarang kembali ke rumah kediaman, dengan menunjukkan sertifikat mandiri bebas covid-19; (3) dibolehkannya aktivitas olahraga individual dan dimulainya latihan rutin para atlit profesional, termasuk sepak bola; (4) dibukanya pabrik, jasa konstruksi dan lokasi konstruksi, dan perdagangan grosir yang mendukung sektor-sektor tersebut, dengan mengikuti protokol keamanan di lokasi kerja; (5) layanan take away untuk bar dan restoran, namun masih melarang konsumsi di tempat; (6) Taman kota atau lokasi bermain umum boleh dibuka dan diakses, dengan menghindari kerumunan. Namun demikian, perpindahan manusia ke provinsi lain masih dilarang, kecuali untuk alasan bekerja dan kesehatan, demikian juga dengan kegiatan di rumah ibadah.

Beberapa kelonggaran juga akan mulai diterapkan pada tanggal 18 Mei, yaitu dibukanya jasa perdagangan retail, pameran dan museum, serta boleh dimulainya latihan olahraga secara berkelompok. Sekali lagi, pelonggaran ini tetap mematuhi aturan dasar social distancing, dengan tetap menggunakan masker dan menjaga jarak aman. Kemudian pada tanggal 1 Juni, bar, restoran dan salon kecantikan atau tukang cukur untuk umum akan mulai dibuka, dengan tetap memegang prinsip social distancing dan menggunakan masker.
Keputusan ini diambil dengan mempertimbangkan trend berkurangnya jumlah kasus positif per hari, meingkatnya jumlah kasus sembuh per hari, dan berkurangnya jumlah meninggal per hari (foto terlampir).

RENCANA TAHAP 3

Syarat terpenuhinya tahap 3 adalah zero penularan, dimana diperlukan vaksin atau ditemukannya bentuk terapi lainnya, yang hingga saat ini secara ilmiah belum bisa diprediksikan. Proses evolusi ilmiah virus hingga saat ini masih belum jelas, dan masih terlalu umum. Sehingga hal yang mungkin dilakukan saat ini adalah pelonggaran karantina wilayah secara bertahap. Pemerintah pun belum bisa memastikan kapan tahap 2 akan berakhir dan kapan tahap 3 akan dimulai.

“Kita akan mulai melonggarkan lockdown, tapi harus menjaga situasi tetap terkontrol. Setiap region akan memberikan informasi tentang kurva epidemis setiap harinya. Tiga hari sebelum diberlakukannya Dekrit 4 Mei ini, Menkes Speranza akan menunjukkan ambang batas yang harus ditaati sehingga dapat mengevaluasi situasi dan memutuskan bisa membuka atau menutup keran lockdown ini,” ujar PM Conte menutup pernyataan pers-nya.
